  • Santa Clara Family Health Plan offers all kinds of providers, including individuals, groups, clinics and university centers. You can choose from many doctors, hospitals, and pharmacies.
  • You receive comprehensive benefits: doctor visits, prescriptions, hospital, emergency room, immunizations and well-baby care.
  • You can take advantage of extra services and classes: acupuncture, chiropractic services, interpreter services, special diabetes and asthma programs, and free wellness classes fitness activities, recreation opportunities and support groups.

How the Plan Works

  • Pick a doctor – and we’ll help. Your child’s doctor will refer your child directly to a specialist when needed. Female members can see their OB/GYN without a referral. And you can change doctors any time for any reason.

  • We listen. Join our Consumer Advisory Committee!
  • We will arrange for an interpreter for your doctor visits.
  • You can use any of our many pharmacies, including CVS, Walgreens, Rite Aid, Save Mart, Safeway, Target, Costco, and many independent pharmacies.
