Why choose Community Health Plan?
Community Health Plan (CHP) has a large network of physicians, hospitals, pharmacies, and medical professionals to choose from throughout Los Angeles County. We contract with public and private hospitals right in your community which provide eligible children with access to quality health care services. If you choose Community Health Plan, the Community Provider Plan in Los Angeles County, your monthly premium will be less than if you choose another Healthy Families Plan in the county. Premiums ranging from as low as $4 - $21 per child per month to a maximum of $63 per family per month.

- Urgent care centers open until 11:00 p.m., 7 days a week.
How the Plan Works
- You may choose your child’s personal primary care doctor when he/she enrolls.
- You will receive an ID card for medical services and prescriptions.
- Your child’s primary care doctor will coordinate all needed care for your child.
- If you need to change doctors, our Member Services Department will assist you.
- Prescriptions can be filled at any of our many pharmacies in your community.
- Questions? Call our Nurse Advise Line or the Member Services Department if you need assistance.
