Why Choose Community Health Group?

We have been caring for people in San Diego County since 1982. Our service area also includes Southeastern Riverside County. As a local nonprofit health plan, we reinvest our resources in new programs and services. If you choose Community Health Group, the Community Provider Plan in
San Diego County, you will pay less than if you choose another Healthy Families Plan in the county. Our Plan is available in San Diego County from $4 - $21 per child per month, up to a maximum of $63 per month for all children in the family. Our Plan in Riverside County is available from
$7 - $24 per child per month, up to a maximum of $72 per month for all children in the family.

Our members have access to:

  • Member Services 24 Hours a Day, 7 Days a Week, by calling 1-800-224-7766.
  • Telephone Advice Nurse is available from 5:00 am to 8:00 pm seven days a week.
  • Many primary care doctors and specialty doctors.
  • Many pharmacies, including Rite Aid, CVS, Longs, Wal-Mart and many neighborhood pharmacies.
  • Most local hospitals, including Children’s Hospital, Sharp Healthcare, Alvarado Hospital and Scripps Hospitals.
  • Multilingual health education services and materials.

How the Plan Works

  • You may choose your child’s doctor or we can help you find one who best meets your needs.
  • You will receive a welcome telephone call during your first effective month.
  • You will receive a welcome packet with information on our services, a Member ID Card, and a Prescription Drug Card.
  • You can go to any of our pharmacies or order prescription drugs through the mail.
  • Urgent care services are available and are authorized through your child’s doctor or referred by the Telephone Advice Nurse.
  • For emergencies, call 911 or go to the nearest hospital emergency room.
