How the Plan Works
- You choose your child’s doctor—one who meets your needs and has office hours that work for you.
- After picking a doctor, you’ll receive an ID card for medical services and prescriptions.
- If your child needs to see a specialist, your doctor will refer you to one.
- You may change doctors at any time to be effective monthly.

If you choose Alameda Alliance for Health, the Community Provider Plan in Alameda County, your monthly premium will be less than if you choose another Healthy Families Plan in the county.
