How do I report a change of address?

To report a change, call 1-866-848-9166. You can call between 8 a.m. and 8 p.m., Monday through Friday, and Saturday from 8 a.m. to 5 p.m.

Make sure that Healthy Families has your correct billing address and the correct address of your children enrolled in the program. You must notify the Healthy Families Program within 30 days if you have a change of address, because your children may need to transfer to a different health plan.

 

If my income or family size changes, do I submit new information?

You do not have to submit new information until the Annual Eligibility Review (AER), or when you add a person. Once eligibility is established, your children will be covered for a 12-month period (one year), unless a child enrolled in Healthy Families turns 19, is disenrolled due to non-payment of premiums for two consecutive months or you request disenrollment.

If your family income decreases at any time before your AER and you would like us to re-evaluate your child’s eligibility or to see if your premiums can be lowered, call 1-866-848-9166 and ask for a Premium Re-evaluation Form.

Changes in your income or family size may affect your children’s future eligibility for Healthy Families. If your income falls below Healthy Families guidelines, your children may be eligible for free health coverage through the no-cost Medi-Cal program.
